The Comprehensive Continuum of Care (CCC) for Women program provides comprehensive case management services, community-based linkage, and retention services through pre-admission service coordination to reduce barriers to treatment, enhance motivation, stabilize life situations, and facilitate engagement in long-term recovery.
Priority Population
Pregnant and parenting women diagnosed with a substance use disorder who are eligible for pre-admission, concurrent services while engaged in treatment, and after-treatment services who meet clinical and financial eligibility for funding from the Texas Health and Human Services Commission (HHSC).
In order to qualify for the CCC program, individuals must be 18 years old or older and meet one or more of the following criteria. Community Healthcore shall give preference for treatment services in the following Health and Human Services Commission (HHSC) established order of priority:
- Pregnant injecting individuals
- Pregnant substance users
- Injecting drug users
- Individuals identified as being at high risk for overdose
- Individuals referred by the Department of Family and Protective Services (DFPS)
- Individuals experiencing homelessness or housing instability
- All other populations
